    DOSAGES

    Ointment : Apply gently on the spots of leucoderma once or twice a day. Apply the ointment from pigmented area to non-pigmented area i.e. from outside to inside. Expose the area after application of the ointment to sun either before 11 a.m. or after 3 p.m. , starting from 6 minutes with an increment period of 2-3 minutes per week till it reaches to 30 minutes of exposure time. The subsequent exposure has to be maintained at this level till the pigmentation is complete. Oral Drops : Adults – 1 ml. to be taken twice a day after major meals, diluted in 150 ml. – 200 ml. water Children – 0.5 ml. to be taken twice a day after major meals, diluted in 150 ml. – 200 ml. water or as directed by the Physician.

    Recommendations

    • Use Copper rich water after storing it in a copper vessel for at least 8 hrs. • Increase green vegetables, carrot, bottle gourd, beans & pulses intake. • Consume soaked red grains 30-50 gms./ day as an immuno-modulator. • Deworming is necessary in case of worms infestation.

    Restrictions

    • Citrus foods viz. Lemon, Orange, Grapes,Tomato,Pickles, Mango, Fine flour(Maida), Butter milk(Lassi), Apple, Chilli, Urad dal and food containing sour and Vitamin C contents. • Non-Vegetarian diet & fast foods. • Beverages like cold drinks. • Chemical based detergents/soaps.

    CAUTION

    • Formation of blisters is absolutely normal process, it shows that the drug has started working. Don’t panic. If patient feels discomfort, itching on applying the ointment to a part, then he may be advised to mix Jasmine (Chameli) oil to the ointment to reduce its concentration. • Sun exposure needs to be adjusted according to the individual sensitivity. The duration of sun exposure may be reduced in hypersensitive patients or in symptoms of excessive irritation. SIDE EFFECTS • Slight irritation has been reported in some patients but it may happen only at the time of application. TOXICITY Studies have shown that Lukoskin did not show any apparent toxicity / adverse effect on the health.

    INGREDIENT HIGHLIGHT

    1. Ammi majus - Skin Photo sensitizer The seed contains furanocoumarins (including bergapten), which stimulate pigment production in skin that is exposed to bright sunlight, . The plant is widely cultivated in India for these furanocoumarins which are used in the treatment of vitiligo (piebald skin) and psoriasis.

     

    2. Psoralea corylifolia

    - Skin Photo sensitizer Babachi botanically known as Psoralea corylifolia grows as weed. Since generations, it is in use as folk medicine in India . It is mainly used in treatment of leucoderma. In many parts of India , it is under cultivation as medicinal crop. According to Ayurveda, seeds are refrigerant, alternative, laxative, antipyretic, anthelmintic and good for heart troubles.

     

    The monoterpenes and flavonoids isolated from the seeds of Psoralea corylifolia inhibited NADPH-, ascorbate-, t-BuOOH- and CCl (4)-induced lipid peroxidation in microsomes. They also prevented NADH-dependent and ascorbate-induced mitochondrial lipid peroxidation. They showed potent antioxidant activity in microsomes and the inhibition of oxygen consumption induced by lipid peroxidation was time-dependent. Furthermore, it protected human red blood cells against oxidative haemolysis. These phenolic compounds in Psoralea corylifolia were shown to be effective in protecting biological membranes against various oxidative stresses.

      

    3. Calotropis gigantia - This large shrub, which looks like a small tree, sports clusters of waxy flowers that are either white or lavender in color. Each flower consists of five pointed petals and a small, elegant "crown" rising from the center, which holds the stamens. The plant has oval, light green leaves and milky stem. Calotropis procera (Madar) significantly augment the healing process by markedly increasing collagen, DNA & protein synthesis and epithilization leading to reduction in wound area.

     
      

    4. Aloe vera - It is stem less or has a short – stemmed plant which can produce up to a height of about 80 cm to 100 cm that spreads for root sprouts and balancing. Aloe Vera leave is fleshy, lance-shaped and thick and it is forever gray – green and green in color. One can also use aloe Vera as a medication to treat skin problems such as cuts, eczema and burns. Aloe Vera drains some fluid which is beneficial to reduce inflammation and pain. A study has shown that when gel made up of aloe Vera is applied on the severe and moderate burns

     

    then it takes just six days to recover from the wound. In the last few decades clinical studies have produced evidence to support centuries of Aloe Vera anecdotal health claims.     5. Centella asiatica - The whole plant is alterative, cardio-depressant, hypotensive, weakly sedative and tonic. It is a rejuvenating diuretic herb that clears toxins, reduces inflammations and fevers, improves healing and immunity, improves the memory and has a balancing effect on the nervous system. It has been suggested that regular use of the herb can rejuvenate the nervous system and it therefore deserves attention as a possible cure for a wide range of nervous disorders including multiple

    Anti-leucoderma ointment and oral dose The polyherbal formulation was developed in the form of ointment and liquid oral dose, taking into consideration the social stigma, limitation of existing therapies and etiology of disease. The product was evaluated on 100 patients with leucoderma (Localista), on the basis of body part affected, sex wise recovery and recovery according to the age of ailment. Maximum recovery was recorded in thorax-hip affected patients while sex wise recovery was better in females below 40 years of age. Patients with minimum age of ailment showed best recovery.

    ACTIVATES THE TYROSINASE ACTIVITY FOR MELANIN SYNTHESIS

     
    An important herb for the treatment of leucoderma, is introduced into India from Egypt, naturalized in Jammu region. Also found in Himalayas at a height of 4000-6000 fts. It is rich source of furanocoumarins like methosalen & bergaptes.
    Ammi majus (Vishnag) is rich in furanocoumarins well known as photosensitizer for initiation of erythema on the spots of vitiligo by the exposure to UVA radiations or sunlight. Coumarins accelrelate the destruction of sulphydryl groups by photo-oxidation and this may increase the formation of melanin in skin. (Standardization of Botanicals, Vol 2, 2005) Ammi majus (Vishnag) helps in the release of SH-group bound copper as free copper, which activates the tyrosinase activity for melanin synthesis.
    The extracts of tissue cultures of the plant showed antibacterial, antifungal and anthelminitic activity specially against E.coli, Staphylococcus, Salmonella, Trichophyton, etc. A number of clinical studies have been carried out with the plant in the treatment of vitiligo. The plant was used both internally as well as externally for different duration, for the treatment of Leucoderma & other skin ailments.
     
     

    FACILITATE AMINO ACID TRANSPORT ACROSS THE INTESTINAL MUCOSA

     
    Psoralea corylifolia (Bakuchi) helps in the release of SH-group bound copper as free copper, which activates the tyrosinase activity for melanin synthesis.
    Psoralea corylifolia (Bakuchi) contains furanocourmarins like psoralen. Which stimulates skin to produce melanin pigment when exposed to sunlight & its seeds are believed to facilitate amino acid transport across the intestinal mucosa. is also a rich source of furanocourmarins, thus it also works as photo-sensitizer for the initiation of erythema on the spots of vitiligo. Furanocourmarins initate the transformation of DOPA to melanin under the influence of UV Light or sunlight. Oxidation of the sulphydryl groups may be responsible for the pigmentation as it as proved by the fact that epidermal tissues contains free sulphydryl groups, which bind the copper required for the function ot tyrosinase.
     

    SUPPLEMENTS COPPER AND L-DOPA

    Mucuna preuriens (Kaunch) is rich in L-DOPA (L-3,4- dihydroxyphenylalanine) and copper. Since L-DOPA is a precursor of melanin and is easily converted to melanin.

    Skin photosensitization is a free radical mechanism, which creates heat sensation & blister formation. This effect has been subsided by addition of Calotropis procera (Madar) & Aloe vera (Kumari).They exerts anti-inflammatory effects mainly be inhibiting histamine and partly by inhibiting PGE2, which further reduces irritation & hence blister formation.     CLINICAL STUDY
     
      

    DARL (Defence Agricultural Research Laboratory) invented a poly-herbal product for treating Leucoderma, keeping in view the limitations and drawbacks of available art of its treatment. An attempt was made to develop a formulation for both topical and oral use with objectives that it may overcome the drawbacks of existing therapies and give better efficacy and cure.

     

    The disturbed auto-immune system is responsible for origin and progression of disease. So to check the emergence of new spots, the oral dose is formulated, along with the ointment, which contain herbs to improve auto-immune system.

     
     

    Lukoskin is being evaluated on the volunteers of various age group of patients of different ailment age and extent of disease spread, under the active guidance and supervision of the Allopathic and Ayurvedic doctors from Govt. District Hospitals, Pithoragarh. The product has been distributed to more than 600 patients out of whom 100 patients have been placed under the regular observation. The observation has proved that out of every 100 patient 41 are completely recovered and the rest 59% are in the range of recovery.

     
    Medicinal plants for Lukoskin were received from Defense Agricultural Research Laboratory (DARL), Pithorgarh and toxicity was evaluated at IRTC, Lucknow.
    oxicity tests in animal models are required for safety evaluation and also for planning further studies. In this study acute toxicity studies of alcoholic extracts of the medicinal plants were carried out for the determination of oral LD50 and primary skin irritation.
     

     

    3.1. Animals : Randomly bred Wistar male rats (175-250 g body weight) maintained in the Establishment animal house, were used for the study. For primary skin irritation test female albino rabbits (2.4 +0.2 kg) maintained in the Establishment animal house. 3.2. Alcoholic extracts of medicinal plants : The alcoholic extracts of the medicinal plants were provided by DARL. 3.3. Acute oral toxicity : The rats were fasted overnight (about 18 hrs water allowed). Doses of 800, 1600, 3200 and 6400 mg/kg body weight of the alcoholic extracts were fed to the rats through an 18 gauge oral feeding cannula.
    3.4. Primary skin irritation test : Hair on the back of rabbits were closely clipped. Four areas (each area = 2.5 cm2) on the skin, two on right and two on left were marked. The left side was kept as control. On the right upper side, 0.5 ml of extract of eczema medicinal plant and on the lower right side, 0.5 ml of extract of leucoderma medicinal plant were applied. The skin sites were covered with a gauze patch and impervious soft sheet (Parafilm), and held in place by means of adhesive tape.

    RESULTS

     

    There was no mortality in the alcoholic extracts upto 3200 mg/kg dose. The oral LD50 is above 5.0 g/kg for the alcoholic extracts. The general behavior of the rats was normal.

     
     

    There was no significant difference in feed and water intake of the treated groups compared to the control group. Primary skin irritation test showed that the extracts induced mild erythema on the day of application and there was no effect 24 hr later.

      

    There was no significant change in the percent weight gain compared to the control group except at a dose of 6400 mg/kg of alcoholic extracts of medicinal plant for leucoderma.
